Coughing in children is one of the most common symptoms primarily resulting from respiratory tract disorders, but may also be associated with a variety of extra-pulmonary causes.[1] A thorough medical history is the cornerstone of evaluating pediatric dry cough, helping clinicians identify the underlying cause with greater diagnostic accuracy than conventional assessment alone. Since cough is a protective reflex, medical attention is warranted when it causes significant discomfort, disrupts daily activities, or raises concern about an underlying condition. A structured, step-by-step history should assess cough onset (acute, subacute, or chronic), characteristics, timing, severity, associated symptoms, relieving and aggravating factors, and environmental exposures such as passive smoking or irritants.[2] In children with chronic cough, details such as age of onset, progression, and the distinctive sound of the cough can provide valuable diagnostic clues, including foreign body aspiration, asthma, chronic bronchitis, or upper airway pathology. Careful listening, exhaustive questioning, and sufficient consultation time are essential to assemble clinical clues. History taking, followed by physical examination, chest radiography, and spirometry, forms the foundation for accurate diagnosis and targeted management of pediatric dry cough.
